Notes:A) Sponsors receive a Free event T-Shirt. Additional T-shirts may be purchased for a cost of $15.00 (If available).B) For additional information about our event, please click on the above "Sponsors Instructions" link and print a copy for your records.C) Please send a copy of your logo (.jpg or .png) to Hawkins@Veterans.ooo for our promotional board and flyers.D) When you have completed this form, please click on the "Submit to VSC" button below.E) If a payment is due, please scroll down to the "Pay Here & Reserve Your Sponsors Table(s)" form below and follow the instructions.
Payments to Sponsor the Mayors' Veterans Appreciation Day event can be made in one of three ways:
- Thru PayPal - Just go to the section below and make your selection.
- By Mail - Make your check or money order payable to "Veterans Support Council, Inc." or "VSC" and mail it to 2457 E Washington St, Suite F, Indianapolis, IN 46201 USA. Please make sure you make a note on your check or money order for what your payment is to be used for.
- Drop Off - Bring your cash or check payment or Donation to any MACV or VSC meeting.

For Medical, Physical Emergencies & Suicide Prevention:

- If you are a victim of domestic violence, dial 1-(800) 332-7385 in Indy for help.
For Non-Emergencies:
- If you are a Veteran, or a family member of a Veteran, in search of information on how to get goods or services you/they may need, you can dial 2-1-1 or contact one of the Service Providers or Veterans Organizations listed on our "Resources" page.
- Cellphone users in Marion County may now dial 3-1-1 to reach the non-emergency line for IMPD and other Marion County dispatchers. The non-emergency phone number for callers using landlines is (317) 327-3811.
